授業の目的 ／Class Purpose	この授業の担当者は薬学部所属であり、この科目は、アドバンス科目である。 この科目は、学部のDPに示す8項目のうち、1,3,4を目指す。 ディプロマ・ポリシー（学位授与の方針） Aim: To provide a comprehensive understanding of ambulatory care, enhanced pharmacy services (chronic care management, annual wellness visits), and expanding community-based pharmacy practice to improve patient outcomes. Objectives: 1. Discuss examples of expanded pharmacy services provided by pharmacists in the US. 2. Identify the role and responsibilities of pharmacists in different healthcare settings 3. Understand challenges and opportunities for pharmacists billing for services in the US Healthcare system 4. Advocate for expanded roles for the profession of pharmacy in healthcare.
到　達　目　標 ／Class Objectives	各回を通じて，以下の8項目を到達目標とする。 JB-(3)-①-1 科学、医療に関連する英語の代表的な用語を列挙し、その内容を説明できる。 JB-(3)-①-2 科学、医療に関して英語で書かれた文章を読んで、内容を説明できる。 JB-(3)-②-3 科学、医療に関連する英語の代表的な用語、英語表現を列記できる。 JB-(3)-②-4 科学、医療に関連する簡単な文章を英語で書くことができる。（知識・技能） JB-(3)-③-1 英語の基礎的音声を聞き分けることができる。（技能） JB-(3)-③-2 英語の会話を聞いて内容を理解して要約できる。（技能） JB-(3)-③-3 英語による簡単なコミュニケーションができる。（技能・態度） JB-(3)-③-4 科学、医療に関連する代表的な用語を英語で発音できる。（技能） AD-B-⑤-2 諸外国における薬剤師の活動分野について、日本と比較しながら説明できる。 - Describe the potential opportunities for pharmacists to help patients in the outpatient setting with chronic disease management - Discuss the roles and responsibilities of ambulatory care pharmacists in the United States - Describe how pharmacists can use medication reconciliation in the care of patients - Summarize technologies used in the management of chronic diseases in the outpatient setting - Counsel a patient on common technologies used to monitor chronic diseases in the outpatient setting

授業時間外に必要な学修内容・時間 ／Required Work and Hours outside of the Class	事前学習として、事前に配布する講義資料を読んでおくこと（目安として1時間）。
提出課題など ／Quiz,Report,etc	1. Develop a mock business plan creating one expanded pharmacy service 2. Identify 3 barriers to expanding roles of pharmacists in healthcare 3. Participate in group discussions and case study analysis.
成績評価方法・基準 ／Grading Method・Criteria	Multiple choice examination and reflective writing 8回全ての授業に出席すること。 成績評価：各授業ごとの小テスト（100％）

参考図書 ／Reference Books	DiPiro’s Pharmacotherapy: A Pathophysiologic Approach, 12th Edition. Please note that students do not need to purchase for this course
